For pupils
All primary school SENCOs have the Communication, Learning and Autism Support Service (CLASS) transition booklets to support pupils. There are three booklets to support transition in KS1 to KS3.
The booklets are not just for children who need additional support, you can use them with all children.
For professionals
Vulnerable Learners Protocol
The protocol is to ensure a successful transition into post-16 for vulnerable young people. They need additional planning, support or monitoring to maximise their potential. The protocol outlines the responsibilities and activities for supporting them.

Schools and further education providers should work together to identify vulnerabilities and ensure that additional planning is in place.
