  • Transition Roadshows for staff to explore resources and strategies.
  • Term 1 reviews in secondary schools to identify and address early concerns.
  • Named SEND PSS Advisers for each secondary school to liaise with primaries and attend transition meetings.

CLASS+:

• • Coffee & Chat sessions and workshops for parents/carers on transition themes.

INCAP:
