5. What to do if a child requires further support

If you think a child/young person requires further support and is not making progress despite the provision of additional support, you can request an Education, Health and Care (EHC) needs assessment.

Please ensure you have looked at the SEN Matrix Guidance and statutory assessment information sheet before completing a request for an EHC needs assessment (statutory assessment). 

To request an EHC needs assessment you should complete the relevant form below and submit it, along with additional evidence, via the ISEND Front Door.
